How to get support coordination added to your NDIS plan

The National Disability Insurance Scheme (NDIS) offers a wide range of supports to help participants achieve their goals. One of the most valuable is support coordination, which helps you understand your plan, connect with the right providers, and work through challenges as they come up. If you have support coordination funded in your plan, here’s how to actually access it and start putting it to work.

What is support coordination?

Support coordination is a funded NDIS service designed to help you turn your plan into action. A support coordinator works with you to:

  • Understand your NDIS plan and funding
  • Connect with the right services and providers
  • Navigate challenges, like waitlists or eligibility requirements
  • Build the skills and confidence to manage your supports more independently

Support coordination is funded at different levels depending on your needs:

  • Level 1: Support Connection — short-term help to understand your NDIS plan, connect with providers and set up your initial supports.
  • Level 2: Support Coordination — ongoing help to understand your plan, coordinate multiple supports, solve problems and build your confidence to manage your supports.
  • Level 3: Specialist Support Coordination — more specialised guidance for participants with complex or higher-risk circumstances, including coordinating across multiple services and managing significant changes.

Check your plan for support coordination funding

Before you get started, check your NDIS plan to confirm you have support coordination funding, and how much. This is usually listed under “Capacity Building — Support Connection.” If support coordination isn’t currently in your plan, you can raise this at your next plan review. Be ready to explain the challenges you’re facing managing supports on your own, and how a support coordinator would help you work towards your goals.

Steps to access support coordination

  1. Choose a support coordination provider: You have choice and control over who supports you. You can ask your planner for suggestions, search the NDIS provider directory, or go directly to a registered provider like CoAct.
  2. Get in touch and make a referral: Reach out to your chosen provider by phone, email, or an online referral form. Let them know you have support coordination in your plan and a bit about what you’re looking for.
  3. Have an initial conversation: Your provider will contact you to understand your needs, confirm your consent, and explain how they can help. This is a good chance to ask questions and make sure they’re the right fit for you.
  4. Get matched with a support coordinator: You’ll be connected with a support coordinator based on your goals, location, and preferences, and arrange a time to meet, whether that’s in person, by phone, or via video call.
  5. Start putting your plan into action: Together, you’ll go through your NDIS plan, identify the supports and services you need, and start connecting with providers. Your support coordinator will keep checking in with you and adjust your supports as things change.

Tips for getting the most out of support coordination

  • Be clear about your goals and what you’d like support coordination to help you achieve
  • Let your support coordinator know about any barriers you’re facing, so they can help address them
  • Ask questions if anything about your plan or funding is unclear
  • Use the experience to build your own skills and confidence in managing supports over time
