NDIS Support Coordination

NDIS Support Coordination

Finding the right support coordination makes a real difference to how well your NDIS plan works for you. CoAct Support Coordination takes the time to understand what matters to you and works alongside you to make it happen.

What is support coordination?

Support coordination is an NDIS-funded service that helps you get the most out of your plan. A support coordinator works with you to understand your funding, connect with the right providers, and make sure your supports are set up to help you reach your goals.

It is not about making decisions for you — it is about making sure you have the information, connections and confidence to make decisions yourself.

What a CoAct support coordinator can help you with

Every participant’s situation is different. Our coordinators take a practical, personalised approach to make sure your supports work for your life.

We can help you:

  • Make sense of your NDIS plan and what your funding covers
  • Find and set up services and providers that suit your goals and preferences
  • Sort out problems when services are not working or your needs change
  • Prepare for plan reviews with clear evidence of what is and is not working
  • Build your confidence and skills to manage your own supports over time

Who we work with

Our support coordination services are available to NDIS participants in Brisbane and surrounds and Sydney, including:

  • Participants who are new to the NDIS and need help getting started
  • Participants who want to change their current support coordinator
  • People whose circumstances or support needs have recently changed
  • Participants with funding for Support Coordination (Level 2) in their plan

We also work alongside families, carers and nominees — with the participant’s consent — to make sure everyone is informed and supports are running smoothly.

Our services

Support coordination (Level 2): Practical, day-to-day coordination to help you understand your plan, connect with providers, and keep your supports working well together.

Specialist support coordination (coming soon): For participants with more complex needs, we provide additional guidance and intensive coordination to help manage higher-level challenges safely and effectively.
