Refer a participant to CoAct Support Coordination
CoAct Support Coordination is a trusted partner for NDIS participants
CoAct Support Coordination is a new service helping NDIS participants get the most out of their plan, with the right supports in place and everything working well together. We’re a registered NDIS provider for support coordination.
We provide reliable, participant-centred coordination that supports choice, builds capacity, and keeps supports aligned to individual goals.
We support NDIS participants to:
- Understand and use their plan effectively
- Connect with appropriate supports and services
- Navigate changes, challenges, or service gaps
- Build confidence and independence over time
We work alongside participants and, with consent, their families, carers, and broader support networks.
