Support Coordination Information for parents and carers
CoAct Support Coordination is here for parents and carers too
CoAct Support Coordination is a new service from a registered NDIS provider, helping participants get the most out of their plan with the right supports in place and everything working well together.
We understand that parents and carers play an important role in the lives of many participants. With the participant’s consent, we are happy to work alongside you to make sure supports are coordinated, communication is clear, and nothing falls through the cracks.
We support NDIS participants to:
- Understand and use their plan effectively
- Connect with appropriate supports and services
- Navigate changes, challenges, or service gaps
- Build confidence and independence over time
We work alongside participants and, with their consent, their families, carers, and broader support networks.

Our support coordination services
Support coordination (Level 2): We support participants to understand their plan, connect with providers, and coordinate supports effectively.
Specialist support coordination (coming soon): For participants with more complex needs, we provide extra guidance to coordinate supports safely and effectively.

Why parents and carers choose CoAct Support Coordination
When an NDIS participant is supported by CoAct, parents and carers can feel confident that their person is receiving consistent, person-centred support. We prioritise clear communication, strong safeguarding practices, and a genuine commitment to participant choice and control.
- Reduced coordination burden for parents and carers
- Clear and regular communication (with participant consent)
- Strong consent and safeguarding practices
- Support to navigate changes, crises, or service gaps
- A team that works alongside you, not around you
Our approach is participant-centred and grounded in CoAct’s organisational philosophy. With participant consent, we coordinate the people and services around them — including providers, family, carers, and community supports — to ensure:
- Clear communication across everyone involved
- Defined roles and responsibilities
- Supports that are aligned and working together
We focus on practical coordination, problem-solving, and maintaining continuity of support.
